Arimidex (anastrozole) is an aromatase inhibitor (AI) sometimes used off-label in Testosterone Replacement Therapy (TRT) to manage high estradiol levels. It works by blocking the enzyme that converts testosterone into estrogen, preventing side effects like gynecomastia, mood swings, and water retention. 
Purpose: It is used to lower high estradiol (E2) levels in men on TRT, not as a standard, one-size-fits-all addition, as many men on TRT do not need it.
Dosage: Typical protocols involve low, intermittent doses, such as 0.05 – 1.0 mg every 1–3 days, adjusted based on bloodwork and symptoms.
Risks of Over-suppression: Reducing estrogen too much (crashing) can lead to side effects like low libido, fatigue, joint pain, and erectile dysfunction.
